(Primary Location for Job Responsibilities) Our Kennesaw campus is located at 1000 Chastain Road NW, Kennesaw, GA 30144.
Our Marietta campus is located at 1100 South Marietta Parkway, Marietta, GA 30060.
Department Information
DEPARTMENT SPECIFIC TASKS AND RESPONSIBILITIES:
Provides case management services to individuals with varying needs, such as assessment completion, 1:1 touchpoint with clients, and making referrals to appropriate campus and community resources
Develops a holistic treatment plan to address each client's specific needs
Manages client documentation and track client
Assists in the creation of psychoeducational materials, such as training and resource materials
Gathers qualitative data through interviews with Military Veterans
Attends outreach events to increase awareness among and collaboration with community partners regarding current and future projects
DEPARTMENT SPECIFIC KNOWLEDGE, SKILLS, AND ABILITIES*:
Knowledge of military, veteran, and military family health, well-being, and culture
Knowledge of first responder (e.g., police, fire/rescue, EMS) health, welling, and culture
Knowledge of academic subjects pertinent to health and wellbeing (e.g., psychology, public health, human performance, medicine, biology, sociology)
Ability to integrate perspectives from multiple academic disciplines
Knowledge of interventions to improve mental and behavioral health
Job Summary
Responsible for developing and implementing action plans to assess and manage the individual needs of students of concern, ensuring they are connected to the appropriate levels of care. Provide problem-solving support, resources, and advocacy for external challenges that are impacting a client.
This position is considered limited-term staff since it utilizes contingent funding. Continued employment is contingent upon availability of grant funding for this position.
Responsibilities
KEY RESPONSIBILITIES*:
1. Provides case management and related support services for clients on a wide variety of issues of concern, including assessment and immediate and long-term assistance/referrals
2. Provides direct clinical services to assigned clients; meeting 1:1 initially and on a regular basis to develop holistic success plans based on their unique circumstances. Tracks progress toward client's goals.
3. Develops comprehensive and supportive action and follow-up plans that identify and address the immediate needs of clients of concern and connect them with the appropriate resources.
4. Conducts follow-up meetings with clients to determine the success of and/or need for additional referrals or interventions.
5. Makes appropriate referrals to campus and community resources to ensure clients receive the support necessary to minimize disruptions and assist in resolving challenges impacting their success.
6. Collects, evaluates, documents, and follows up on all information and communications pertaining to client cases and/or referrals. As appropriate, process incoming and outgoing records requests.
7. Assists in training and consulting with internal and external stakeholders, as needed.
8. Provides outreach and consultation to the campus community
9. When needed, assists with required documentation for university academic and financial aid appeals submissions.
Required Qualifications
Educational Requirements
Bachelor's degree from an accredited institution of higher education in Counseling, Psychology, Social Work, Marriage and Family Therapy, or related field
Other Required Qualifications
Current, valid, and unrestricted driver's license
Required Experience
One (1) year of experience, including internships, providing case management services, working in a community mental health context, and/or a crisis response setting.
Preferred Qualifications
Additional Preferred Qualifications
License eligible as independent practitioner in Georgia
Preferred Educational Qualifications
Advanced degree from an accredited institution of higher education relevant to the position
Preferred Experience
Experience in case management in a higher education setting
Experience with conflict resolution, advising, and advocacy
Knowledge, Skills, & Abilities
ABILITIES
Ability to recognize and protect confidential information and exercise judgment, tact, and diplomacy in handling sensitive information and situation
Able to handle multiple tasks or projects at one-time meeting assigned deadlines
KNOWLEDGE
Demonstrated experience interacting with internal and external colleagues and partners
Familiarity with FERPA and HIPPA regulations
SKILLS
Excellent interpersonal, initiative, teamwork, problem-solving, independent judgment, organization, communication (verbal and written), time management, project management, and presentation skills
Proficient with computer applications and programs associated with the position (i.e., Microsoft Office suite)
Strong attention to detail and follow-up skills
Strong customer service skills and phone and e-mail etiquette
